55/M Pyrexia under evaluation with cervical laminectomy 2 months back

 Chief complaints:   Patient came with cheif complaints of burning type of sensation of whole body since 1 week. Fever since 1 day HOPI:  Patient was apparently asymptomatic 2 months back after which he had an RTA during which he had cervical cord stenosis and was operated with cervical cord laminectomy i/v/o weakness of both upper and lower limbs. After which patient was on physiotherapy and power improved gradually.Now patient had h/o burning sensation and numbness of whole body( paresthesia). He also have complaints of fever since today afternoon associated with chills , relieved on medication. H/o of  burning micturition since 4-5 days  No H/o decreased sensations No h/o bowel and bladder incontinence. 64yr old male with renal calculi

 July 27/7/2022 A 64 yr old male ,resident of chotuppal came with chief complaints of pain in the right loin, vomiting and burning micturition since one month HOPI Patient was apparently asymptomatic 15 yrs back.Then he developed pain in the right loin and was diagnosed with renal calculi(bladder) and was operated and then 3 yrs later again he developed bilateral loin pain and was diagnosed with bilateral renal calculi (ureters) and was operated with a gap of one month. Then 3 yrs later he developed a midline swelling approx 3 cm below umbilicus and diagnosed with abdominal hernia and a mesh was placed. A 65 yr old male with pyrexia secondary to UTI

 A 65 yr old female from Narketpally labourer by occupation came to the OPD with cheif complaints of Fever since6 days and burning micturition since 6 days HOPI: patient was apparently asymptomatic 6days back then she developed fever which is insidious in onset and intermittent in nature which aggravated during night and relieved on medication associated with generalised weakness and not associated with chills , rigors , cough, cold, headache, night sweat. H/O  burning micturition  No H/O sore throat ,vomitings  since 6days difficulty in passing urine, decreased urination  ,unable to pass urine without strain.
